Code review fixes:

double miss counting, asyncio.Lock for PG init, LIMIT on queries,
__import__ anti-pattern → _utcnow()
- P2: InMemory TTL cleanup, embedding preservation on put(),
data TTL = max(exact_ttl, semantic_ttl)
